What do I need to become a new patient at Community Health Centers of Greater Dayton (CHCGD)?
Just contact the Health Center of your choice for an appointment. You may also simplify the registration process by completing our
Patient Registration Form prior to your first visit. This form can be downloaded from our Forms page or completed at the Health Center on your first visit.

Do I need an appointment or can I just walk-in?
Corwin Nixon and
East Dayton Health Centers see patients by appointment only.
The Charles Drew Health Center prefers to see patients by appointment, but will accept walk-ins at 7:45 am and 12:45 pm. Only established patients may be seen as walk-ins, new patients must make an appointment. To reduce wait-time, patients are encouraged to schedule an appointment..

Do you take patients who are uninsured?
We do accept uninsured patients however we do not provide free services. The services for uninsured patients are provided at a discount. The amount of the discount is based on a sliding fee scale which is determined based on family size and
documented household income. The

How do I contact my physician after hours? Simply call the office and you will be given instructions for contacting the physician on-call If you are experiencing an emergency situation, hang up and dial 911. Physicians are on call for urgent situations only. If you are calling to cancel an appointment, to request a prescription refill or for any other non-urgent issue, please call during business hours.
