Effective January 1, 2022, the management of the primary care portion of Victor Cassano Health Center located at 165 S. Edwin C. Moses Blvd., will transfer from Community Health Centers of Greater Dayton to Primary Health Solutions.

Importantly, you also have the choice to remain with Community Health Centers of Greater Dayton at one of our other locations listed below.
Your records are already available at each of our centers so you do not need to take any action other than scheduling your next appointment.
